Chain of Custody Page (Request for Analysis)

A Chain of Custody (COC) document must accompany the request for analysis on samples delivered to the laboratory. It is important to have the following on a COC:

1. Client Name, Address, Telephone, Fax Number, Cell Phone

2. Project Name, Number and Location

3. Turn Around Time (TAT)

4. Method of Analysis

5. Sample Numbers

6. Relinquishing Signature
